GIFT COUPON’S ORDER FORM

Rules of Gift coupon usage:

• Gift coupon is not convertible into money.
• Gift coupon confirms the right of gift coupon holder to use the facilities of rehabilitation centre “Atgaiva tau” for the sum, indicated in the gift coupon. If the gift coupon holder requests services for the bigger sum than it is indicated in a gift coupon, he can pay the resulting difference at reception.
• Gift coupon is valid for three months starting from the date of sale. If the gift coupon hasn’t been used during the period of its validity, it is not valid any more and money is not refunded.
• The holder of gift coupon must order the services in advance.
• Services are provided only when the gift coupon is provided.
